An official transcript is a legal document of completed courses and grades. An unofficial transcript is a document of completed courses and course progress hours. Students may obtain official copies of their academic transcript from the Records Office. All outstanding financial obligations must be up-to-date prior to transcript application.

How to obtain a transcript:

1. Fill out and submit the online request form.
2. Official transcripts cost $2.00 each. Payment may be submitted
   • online using the Student Portal (you will need your student ID)
   • or by calling the Cashier at 801-627-8313

OR

1. Go to Student Services  SS123 during the following hours:  Monday/Thursday  8:00 a.m. to 5:00 p.m.;  Tuesday/Wednesday 8:00 a.m. – 6:00 p.m.; Friday 8:00 a.m.  to 12:00 p.m.
2. Fill out an application in the Records Office.
3. Official transcripts cost $2.00 each.

Things to Keep in Mind:

• Allow ten working days for processing transcripts.
• Transcripts picked up in person require photo identification.
• No phone or email requests for transcripts are allowed.
• Transcripts requested by mail or fax should include: student name, social security number, date of birth, student’s signature, approximate dates of attendance, and the complete address where the transcript should be sent.
• Transcript requests by anyone other than the student must be accompanied by a written release from the student. The person receiving the record will be asked to show photo identification.
• The transcript request form is available in the Records Office or online.
