Transcript Request

The Transcript Request Form is available in the Records Office or online. This form is required to request transcripts. In accordance with the Family Educational Rights and Privacy Act, transcript requests cannot be completed by phone or email. Allow 5 to 10 working days for transcript processing. Transcripts picked up in person require photo identification. Transcript requests by anyone other than the student must be accompanied by a signed release from the student. The release must specify the student is authorizing the release of academic records and include the name of who is authorized to pick up the records. The person receiving the records will be asked to show identification.

An official transcript is a legal document of completed courses and grades. An unofficial transcript has the same information as the official transcript but is not printed on official transcript paper nor sealed in an envelope as an official transcript. All financial obligations to the college must be paid in full prior to any record release.

Unofficial transcripts and other progress/attendance verifications may be printed from the online student portal. After logging in, students can access the unofficial transcript in the “Reports” section.

  1. Fill out and submit the online request form.
  2. Official transcripts cost $2.00 each. Payment may be submitted
    • online using the Student Portal (you will need your student ID)
    • or by calling the cashier at 801-627-8313

Hours: The Records Office is frequently closed due to campus meetings and other appointments during listed office hours so we recommend that you call 801-627-8392 to schedule an appointment to guarantee we are available to serve you. The usual Records Office hours are Monday & Tuesday 8:30 a.m. – 6:00 p.m.; Wednesday & Thursday 8:30 a.m. – 5:00 p.m.; Friday 8:30 a.m. – 12:00 p.m. The office is closed Monday-Thursday from 1:30-2:30 p.m.
