What you’ll need to start your benefits:
- An appointment with Veterans Specialist (Call (801) 627-8309)
- Certificate of Eligibility from VA. Need a replacement? Call (888) 442-4551
- DD-214 Member 4 (If lost, request one HERE)
- College transcripts (if you have any)
- Change of Place/Program, can be found HERE (Turn in to the Veterans Specialist)
- Completed OWATC VA Guidelines form
- Completed OWATC Prior Credit Form
