What you’ll need to start your benefits:

  • An appointment with Veterans Specialist (Call (801) 627-8309)
  • Certificate of Eligibility from VA. Need a replacement? Call (888) 442-4551
  • DD-214 Member 4 (If lost, request one HERE)
  • College transcripts (if you have any)
  • Change of Place/Program, can be found HERE (Turn in to the Veterans Specialist)
  • Completed OWATC VA Guidelines form
  • Completed OWATC Prior Credit Form
